| Child Asthma Action Plan (0-5 years) - developed by RAMP |
In 2001, the Regional Asthma Management & Prevention (RAMP) Initiative, and the Childhood Asthma Initiative, a program funded by the California Children and Families Commission for children ages 0-5, developed these Asthma Action Plans for children (ages 0-5). They are available in color and triplicate in English, Spanish, Chinese, and Vietnamese and can also be downloaded below.
A plan for ages 6-adult is also available in English, Spanish, Chinese, and Vietnamese.
