Creating a Medical Home for Asthma for clinic-based interventions
A new research-based asthma intervention is now available for widespread implementation in communities. "Creating a Medical Home for Asthma" (CMHA) is a clinic-based intervention that is can change the way all clinic staff interact with the patient with asthma. CMHA is an asthma management program that encourages public health clinics to implement a team-based approach to pediatric asthma management and care. The team-based approach encourages all clinic personnel to work together as a team to effectively deliver patient-centered asthma management and care. All of the information and materials needed to implement this intervention are available at no cost through the New York City asthma website at http://www.nyc.gov/html/doh/html/cmha/index.html.
